📋 Description

• Utilize collaborative process of assessment, planning, implementation and evaluation to engage, educate, and promote/influence members' decisions related to achieving and maintaining optimal health status • Work from home, requiring reliable phone and hard-wired internet service • Work schedules determined based on business needs • Must be reachable via phone, videoconference, and email during work hours • Maintain confidentiality (HIPAA compliant) • Meet additional reasonable requirements set by the Leader or Company

🎯 Requirements

• Active unrestricted RN compact state licensure • 3+ years of clinical experience required in: Disease Management, Emergency Room, Critical Care, Medical/Surgical Care, telehealth and/or Oncology • Proficient in Multisystem Navigation • Effective Multitasker • Basic Technical Troubleshooting • Digital Collaboration & Documentation • Highly Organized & Results-Oriented • Metric-Driven Performance • Managed Care experience preferred • Previous coaching experience preferred • Telephone triage nursing experience preferred • Associate Degree required • Bachelor's Degree or Equivalent 3-5 years clinical experience preferred
