📋 Description

• Support the work of Care Management staff • Provide daily leadership and mentoring to Field Nurses and Care Coaches (IDT) • Deliver Family Care benefits in compliance with the DHS contract and program values • Identify learning needs and arrange mentorship, orientation, and ongoing learning • Assess and coach staff on IDT competencies and performance standards • Review reports, assess processes, and review documentation • Assess utilization and quality of care management for members • Collaborate across departments to ensure provider-service quality and effectiveness • Educate community partners • Participate in hiring by selecting, orienting, and training associates • Facilitate in-person unit meetings and attend quality visits as needed • Support IDT staff covering Jackson and Trempealeau Counties • Travel at least once per month, with potential for additional travel • Report to the Associate Director, Care Coaching; manage approximately 11–15 direct reports

🎯 Requirements

• Bachelor's degree in human services or a related field or associate degree in nursing • 1 year of leadership experience in the human services field • 3 years of experience working in the Family Care program • Reside within 1 hour of either Black River Falls or Blair, WI • Preferred: 5 years of experience working with individuals with intellectual/developmental disabilities, physical disabilities or frail elders • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
