📋 Description

• Lead teams of nurses, care coordinators, and behavioral health professionals • Supervise, direct, and evaluate a diverse group of health care professionals • Monitor case management activities and implement system-view recommendations • Develop audit plans to ensure compliance with performance metrics • Conduct interviews, hiring, mentoring, and evaluation of staff • Onboard new associates and manage performance • Conduct field-based interactions for care team oversight

🎯 Requirements

• Must reside in the state of Illinois in Cook County or within specified ZIP codes • Active, unrestricted Registered Nurse (RN) license, LCSW OR LCPC in Illinois • Five or more years of professional experience in health care/care management • Two or more years of leadership experience • Ability to use electronic information applications/software programs • Proficiency in analyzing and interpreting data trends • Ability to travel throughout Illinois • Advanced degree in nursing or business health field (preferred) • Certified Case Manager (CCM) (preferred) • Previous experience in managed care field (preferred) • Knowledge of community health and social service agencies (preferred) • Bilingual or multilingual in required languages
