📋 Description

• Identify resources and community support available to families or caregivers • Provide information, referrals, and non-clinical assistance with linkage to medical, psychosocial, and basic-needs services • Educate and guide families, providing supportive tools to navigate systems and access needed services • Assist with accessing programs and supports addressing psychosocial needs within Imagine Pediatrics and the broader community • Advocate with community agencies on behalf of clients and families to decrease service gaps and remove barriers • Connect with internal staff and providers to assist with system navigation, including eligibility, pediatrics, behavioral health, and public health • Assist in developing the community resource library and expanding knowledge of available services • Participate in scheduled consultations with an interdisciplinary team regarding administrative and non-clinical resources • Perform other duties as assigned

🎯 Requirements

• Bachelors degree or higher in healthcare, behavioral health, or social services related field preferred • Certification as a Community Health Worker highly desired • Familiar with navigating social service agencies • Must have reliable transportation, a valid Driver's License, and ability to produce a copy of current auto insurance certificate • Experience with resourcing within a patient or family facing role • Experience with an electronic health/medical record strongly preferred • Bilingual Spanish preferred • Candidates must reside in the tri-state area • Ability to work 8:00am-5:00pm Monday-Friday
