📋 Description

• Working collaboratively with a physician-led health team. • Completing Nutrition Assessments using the LifestyleRx tools and systems. • Meeting with individuals via telephone or video calls from home. • Creating nutrition goals with individuals and providing ongoing support and accountability in cooperation with a health-care team. • Scheduling nutrition and coaching sessions. • Documenting appointments in our electronic medical records system. • Monitoring digital tracking used with individual health programs. • General help involved while working in a clinic setting.

🎯 Requirements

• Registered Dietitian • Experience working in a clinic or health-care setting. • Experience working collaboratively with a health-care team.
