📋 Description
• Act as the primary point of contact for prescribers and clinical teams to enroll patients and coordinate services. • Provide patient education (disease information, medications, and side-effect management) and deliver injection training for home, clinic, or virtual administration according to patient preference. • Support treatment adherence through scheduled follow-ups and promptly communicate clinical updates to physicians and clinical staff. • Maintain concise, accurate documentation and perform chart reviews as needed to support reimbursement.
🎯 Requirements
• Bilingual: Fluent in French and English, both spoken and written. • Nursing license in Ontario or Quebec (or both). • Registered Nurse (RN) designation strongly preferred; Registered Practical Nurses (RPN)/Licensed Practical Nurses (LPN) and Nurse Practitioners (NP) will be considered. • Minimum of 2 years of relevant experience (or an equivalent combination of education and experience).
