• Provide drug information answers to patients, families, pharmacists, nurses, physicians, medical students, pharmacy students, and hospital personnel • Approve new drug orders, including chemotherapy verification, informed consent checks, and non-formulary requests • Screen for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food, drug-disease state, and drug-laboratory interactions • Coordinate patients' pharmaceutical needs in patient care areas • Approve and distribute medication orders • Ensure appropriate dose, route, frequency, and duration of therapy • Review patient medication profiles for interactions, duplicate therapy, allergies, and appropriate regimens • Coordinate pharmaceutical needs with physicians and nurses • Supervise Pharm.D. students and pharmacy technicians • Teach undergraduate and graduate students • Provide educational programs for physicians, nurses, and pharmacy staff • Participate in relevant hospital and departmental committees
• Licensed or eligible for pharmacist licensure in Ohio • Advanced degree (Pharm.D./MS) preferred • One year of residency training and/or equivalent experience preferred • Successful completion of a background check required for final candidates • Drug screen or physical may be required during the post-offer process • Ability to work varying shifts • Ability to provide care appropriate to adult, adolescent, geriatric, and neonatal patients • Knowledge of medication dosing and pharmaceutical needs relative to disease process and patient age
