📋 Description

• Provide real-time call queue support for Care Advice Line functions not requiring RN licensure or independent clinical assessment • Support the Care Advice Line call queue operating after hours and 365 days per year • Answer incoming calls promptly according to SOP and training guidelines • Communicate effectively with patients, caregivers, peers, and healthcare providers • Identify patients, caregivers, and their primary purpose and need for calling • Triage and transfer calls and cases to RNs, providers, team members, and 911 • Chart accurately and efficiently in the EMR and applicable applications • Collaborate with the integrated care team to support patient care coordination • Answer and resolve business office inbound calls related to population health, care coordination, or other clinical activities • Motivate patients to be proactive about their health • Provide education and assistance with patient self-management goals • Make referrals to internal care teams, payer programs, community resources, home care, and disease management programs • Assist with scheduling PCP, Virtual Clinic, or specialist appointments • Perform other assigned duties and projects

🎯 Requirements

• Relevant clinical experience • Patient and provider communications skills • Technical competency and educational background to support the Care Advice Line team • Ability to constantly operate a computer and other office productivity machinery • Ability to read and use close vision, including the ability to do so on a computer screen • Ability to frequently communicate and exchange information • Ability to frequently adjust focus • Minimum remote-work internet speed of 5 MBPS download and 3 MBPS upload • Ability to work the specified weekend, weeknight, and holiday shifts
