📋 Description

• Serve as the first point of contact for patients and their families • Guide older adults and caregivers through the intake process • Provide patient support, care coordination, and emotional guidance • Help patients navigate entry into care • Match patients with the right therapist • Support patients throughout the intake and care-navigation journey • Help older adults and families feel supported, understood, and reassured

🎯 Requirements

• Candidates must be based in Canada • Full-time availability • Empathetic and clear communication • Active listening and ability to handle difficult or emotional conversations • Highly organized and detail-oriented • Ability to manage multiple patients at different stages • Comfortable learning Google Workspace, EHR platforms such as Healthie, and CRMs such as HubSpot • Ability to learn new software quickly • Interest in mental health and healthcare access • Comfortable working Eastern Time Zone hours • Willingness to work one of the listed shifts: 9a–6p ET or 12p–9p ET, across the specified weekly schedules • Experience in healthcare, customer service, or administrative support is welcome but not required
