📋 Description

• Serve as the first point of contact for patients and their families • Guide older adults and caregivers through the intake process • Provide patient support, care coordination, and emotional guidance • Help patients match with the right therapist • Support patients throughout their transition into care • Ensure patients feel supported, understood, and reassured • Take initiative and help shape key processes within a small, agile team

🎯 Requirements

• Based in Canada • Strong empathy and clear communication skills • Active listening and ability to handle difficult or emotional conversations • Highly organized and detail-oriented • Ability to manage multiple patients at different stages • Comfortable learning Google Workspace, EHR platforms such as Healthie, and CRMs such as HubSpot • Ability to quickly learn new software • Interest in mental health and healthcare access • Experience in healthcare, customer service, or administrative support is welcome but not required • Willingness to work Eastern Time Zone hours • Full-time availability
