Service Coordination Unlimited is a nonprofit organization that provides personalized service coordination and supports coordination to individuals with physical disabilities, intellectual/developmental disabilities, autism, and older adults across Pennsylvania. It assists participants in enrolling in and navigating state waiver programs (Community Health Choices, Long‑Term Living, IDD waivers), locates and coordinates person‑centered services, monitors supports, and advocates for independent community living. SCU emphasizes self‑determination, inclusion, and partnerships with state human services to help people live safely and meaningfully in their homes and communities.

📋 Description

• Assess, identify, secure, and monitor services for program participants based on their unique needs • Respond to participant needs through various communication methods • Meet compliance standards and required timeframes for assessments, service plans, visits, calls, and incident reporting • Document each visit and contact in required systems using a detailed, person-centered approach • Provide training and support to participants and consumer employees managing direct care services • Maintain and use internal databases to track participant information and encounters • Help participants obtain accessibility adaptations, specialized medical equipment, non-medical transportation, and other services • Demonstrate company values in all interactions and duties

🎯 Requirements

• Bachelor’s degree REQUIRED; social work, psychology, or related fields preferred • OR at least three years in a social service or healthcare-related setting REQUIRED • Practicum experience is a plus • Experience with people with disabilities or seniors and/or at least one year of LTSS, Service Coordination, or Case Management experience preferred • Excellent communication, customer focus, integrity, job knowledge, dependability, problem-solving, interpersonal skills, productivity, quality, self-development, and teamwork • Proficient in computer systems and relevant technology • Ability to perform reaching, stretching, stooping, sitting, walking, and standing • Ability to lift up to 20 pounds independently • Valid driver’s license and dependable transportation • Must travel throughout Dauphin and surrounding counties
