• Provide timely, expert consultation to primary care Nurse Practitioners for cases outside NP scope, complex cases, diagnostic challenges, and treatment plans • Review patient records, diagnostic tests, and treatment plans and provide comprehensive recommendations • Authorize medical directives for NP implementation when required • Collaborate with NPs and interdisciplinary teams to develop and implement care plans for patients with complex medical needs • Provide clinical education and mentorship to NPs and the team • Support high-quality, evidence-based patient care through consultation and delegation • Support increased access to primary healthcare services in Oxford County, including for vulnerable populations • Advocate for patients’ healthcare needs and coordinate with community resources • Participate in quality improvement initiatives focused on patient care, clinic efficiency, team function, and healthcare outcomes • Contribute to clinical protocols, guidelines, medical directives, and standards of care • Monitor and evaluate patient care protocols and provide feedback for continuous improvement • Work virtually supporting Oxford County MRP Nurse Practitioners • Attend occasional meetings, training, or community outreach events as needed
• Medical Degree (MD) from an accredited medical school • Current licensure with the College of Physicians and Surgeons of Ontario (CPSO) in good standing • Active membership in good standing with the College of Family Physicians of Canada (CFPC) • Minimum of 5 years of clinical experience in a primary care setting • Experience working with vulnerable and marginalized populations, including management of addictions, preferred • Experience working in a multi-disciplinary setting, preferred • Strong clinical and diagnostic skills • Excellent communication, interpersonal, and teamwork skills • Commitment to continuous professional development and evidence-based practice • Proficiency in Electronic Medical Records (EMR) and healthcare coordination/messaging systems • Strong organizational, time management, and problem-solving skills • Ability to work independently and collaboratively within an interdisciplinary team • Cultural competence and sensitivity when working with diverse patient populations • Dedication to patient-centered care and health promotion • Successful completion of a criminal background check and medical screening • Social Insurance Number and, in some cases, a valid work permit required to be eligible to apply
