The Jean Tweed Centre is a Canada-based not-for-profit/charitable organization that provides specialized supports and treatment for women (and their families) experiencing substance use, problem gambling, mental health concerns, trauma, housing instability and related social challenges. With seven locations across Ontario, the Centre offers a trauma‑informed, holistic array of services including intake and stabilization, day programs, residential and live‑in treatment, individual counselling, supportive housing, justice-related programs, and continuing care. It serves thousands of women annually (counselling and care for over 3,000 women each year and broader impact on over 15,000 lives), focuses on recovery and empowerment, and raises funds and awareness through campaigns and events.

📋 Description

• Provide non-urgent primary care assessments in person or by telemedicine to clients. • Promote continuity and effective transitions for clients and families and address ongoing needs. • Provide direct clinical assessments, consultations and primary care within the RN extended class scope of practice. • Determine clients’ needs for urgent or emergent care. • Provide telemedicine assessments to determine the need for emergency room visits or other care. • Establish caring and therapeutic relationships with patients. • Provide individual and group education on health topics, including medications for cravings and withdrawal. • Collaborate with interprofessional teams on care planning and follow-up for high-risk or complex health problems. • Collaborate with community agencies and diagnostic service providers to support comprehensive client services. • Provide education, coaching and support to staff and students. • Support dissemination of research, knowledge and innovative practices. • Collect and analyze data and write reports related to program indicators. • Monitor telemedicine services and identify issues requiring management attention. • Recommend service-gap and safety or quality-of-care improvements. • Conduct responsibilities using harm reduction, trauma-informed, women-centred and anti-oppressive practices. • Follow occupational health and safety requirements, organizational policies and procedures, and support inclusion, equity and continuous professional learning.

🎯 Requirements

• Bachelor’s Degree with Advanced Preparation in Primary Care or Adult Nursing. • Basic Cardiac Life Support certificate required. • Member in good standing with the College of Nurses of Ontario with advanced certification (RN Extended Class). • Minimum of 2-4 years of Clinical experience in a hospital or community health setting, including 2 years caring for clients & families with substance use mental health and gambling concerns. • Experience working in an advanced clinical practice role in primary care or emergency care setting preferred. • Demonstrated in-depth expertise regarding clinical assessment and primary care of women living with substance abuse, mental health and gambling concerns. • Advanced knowledge regarding patient safety, change leadership, project management and evaluation. • Working knowledge with adult education techniques. • Ability to work in self-directed manner using leadership, critical thinking, organizational and problem-solving skills. • Sound interpersonal, team development and conflict management skills. • Sound knowledge of legislation pertaining to substance use & mental health. • Working knowledge of technology and required computer skills. • Experience with Telemedicine use an asset.
