📋 Description

• Complete remote order entry, order verification, and drug verification • Perform transitions-of-care medication reconciliation • Navigate multiple pharmacy information systems • Provide drug information and therapeutic guidance to providers • Assess medication appropriateness, dosing, interactions, and compatibilities • Support specialized populations including pediatrics, geriatrics, pregnant/nursing patients, HIV patients, and patients with renal/hepatic impairment • Maintain knowledge of IV therapies, TPN, cardiac/narcotic drips, IV antibiotics, and chemotherapy • Document interventions and clinical actions accurately and promptly • Communicate with physicians and nurses • Ensure compliance with HIPAA, pharmacy laws, and client policies • Participate in ongoing clinical, PIS, and policy training • Meet productivity and quality standards • Support care teams and ensure safe, high-quality patient care for multiple hospitals

🎯 Requirements

• Bachelor's in Pharmacy required; PharmD preferred • At least 1 year of hospital pharmacy experience strongly preferred • Active and in good standing IL or CA state pharmacist license required • Willingness to obtain additional state licenses (IA, NE, HI) • Strong clinical knowledge and comfort working across multiple systems • Ability to work evenings, overnights, weekends, and holidays • Cable broadband or fiber optic internet connection with at least 10 Mbps download and 5 Mbps upload when working from home • Ability to work across VPNs, automation systems, and multiple pharmacy information systems • Proficiency with Google Suite/Microsoft Office and Snagit • Maintain required licenses, credentials, and system access
