📋 Description

• Conduct initial behavioral health assessments for patients enrolled in the program • Provide regular follow-up through brief, structured interventions focused on symptom management, behavioral activation, adherence support, and care navigation • Administer and track standardized assessment tools to monitor symptom trends and identify patients requiring treatment adjustments • Prepare and present cases during weekly psychiatric consultation, including patients who are new to the program, not responding to treatment, or clinically complex • Implement psychiatric recommendations in collaboration with primary care providers and escalate safety concerns according to established protocols • Coordinate care with primary care teams, existing behavioral health resources, and community services to ensure seamless transitions and continuity across care settings • Document patient interactions accurately and completely in accordance with internal protocols and regulatory standards • Maintain consistent patient engagement and adhere to established clinical workflows and program protocols

🎯 Requirements

• Active and unrestricted license as a Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT) • Holds valid licensure in one or more states with ability to provide care across multiple jurisdictions • Experience working with adult patients with depression and anxiety disorders • Exceptional written and verbal communication skills • Ability to work independently in a remote, technology-enabled environment • Strong organizational skills with attention to detail and follow-through
