📋 Description

• Manage a defined remote patient monitoring and/or chronic care management patient panel • Complete accurate, timely documentation for every patient interaction in accordance with Welby Health protocols and applicable billing requirements • Conduct proactive outreach, device data review, care plan updates, and care coordination • Model clinical engagement, documentation, and conduct standards • Direct assigned team performance, productivity, documentation accuracy, and program compliance • Monitor individual and team KPIs, including clinical engagement hours, unbilled time rates, and remote patient monitoring compliance • Identify and escalate Care Manager and Medical Assistant collaboration breakdowns • Conduct regular coaching sessions grounded in performance data • Manage caseload distribution and escalate capacity constraints or clinical risk patterns • Direct onboarding and provide ongoing training and coaching • Provide patient care coverage during staffing shortages • Track team productivity, patient enrollment, and patient churn • Escalate patient care bottlenecks and emerging risks • Translate leadership direction into team priorities • Contribute frontline clinical feedback to strategy and protocol reviews • Ensure adherence to regulatory requirements, payer guidelines, and Welby Health clinical protocols • Coordinate chart audits and close feedback loops • Maintain billing compliance knowledge and enforce documentation standards • Serve as first escalation point for clinical quality concerns • Maintain relationships with clinic administrators, medical directors, and care coordination staff • Lead client-facing performance reviews, care coordination discussions, and escalation calls • Translate client feedback into operational direction • Develop client success plans and team-level execution plans • Deliver structured performance reporting • Develop corrective action plans • Support expansions, renewals, and new practice onboarding

🎯 Requirements

• Bachelor's degree in nursing or a closely related healthcare field • Active and valid registered nurse (RN) license in good standing • Minimum of three (3) to five (5) years of clinical experience, with demonstrated background in care management, chronic disease management, or a population health program • Prior experience in a lead or supervisory clinical role with direct reports, including accountability for team performance while carrying an individual caseload • Working knowledge of remote patient monitoring, chronic care management, principal care management, or related chronic care program structures and documentation requirements • Demonstrated ability to drive accountability across cross-functional teams without direct authority over all contributors • Demonstrated ability to build and maintain professional client or provider relationships • Ability to manage personal production standards alongside team and client responsibilities concurrently • Exceptional written and verbal communication skills, with the ability to represent Welby Health credibly in client-facing settings • Strong data fluency, with the ability to interpret dashboards, identify trends, and present findings to varied audiences • Familiarity with remote physiological monitoring, EMR/EHR systems, and patient engagement tools • Experience working with Federally Qualified Health Centers or value-based care organizations • Verbal and written proficiency in Spanish
