📋 Description

• Review clinical incidents • Follow standard operating procedures (SOP) to determine root cause, risk, and resolution for incidents • Process interim medical orders as needed and notify physicians of patient needs and changes in condition, such as falls, wounds, injuries, and post-hospitalization • Perform follow-up calls for patients recently discharged from acute hospitalization who are considered high risk for readmission • Communicate with clients, caregivers, coordination teams, payers, field RNs, providers, and key stakeholders to ensure client safety and continuity of care • Follow regulatory guidelines to manage clinical incidents to key deadlines • Share trends and insights from aggregate data to support continuous quality improvement • Collaborate regularly with the assistant director of nursing services to review clinical cases and client needs

🎯 Requirements

• RN, NYS license • Minimum of 2 years nursing experience • Managed care experience preferred • Detail oriented, highly organized, and self-motivated • Strong interpersonal skills – verbal and written communication • Strong clinical judgment and care planning skills • Collaborative with key stakeholders • Reliable computer and noise-canceling headset • Second monitor • Stable internet connection • Backup internet connection • Ability to work Monday to Friday, 09 AM EST to 06 PM EST
