Beschreibung

• Perform comprehensive health assessments of pulmonary rehabilitation patients • Monitor patient progress and adjust rehabilitation plans using exercise tolerance, heart rate, and blood pressure metrics • Provide personalized exercise programming to typically older adults with pulmonary issues through telehealth/video conferencing • Implement exercise plans according to professional and organizational standards • Review patient progress, compliance, and response to exercise • Lead group exercise sessions and update group exercise programming • Document patient information accurately, concisely, and promptly • Provide motivation, emotional support, and cognitive-behavioral strategies • Identify and address barriers to rehabilitation completion and group education participation • Educate patients on cardiovascular health, nutrition, stress management, smoking cessation, dietary changes, stress reduction, and medication adherence • Promote regular program attendance and enroll patients in group education classes • Collaborate with physicians, nurses, physical therapists, dietitians, exercise physiologists, and other multidisciplinary team members • Provide updates on patient progress and challenges to the healthcare team • Maintain documentation and record data for program outcomes, patient goals, and adherence tracking

🎯 Anforderungen

• Bachelor's or Master's degree in Health, Exercise Science, Kinesiology, or related field • CPR and First Aid certification • Required ACSM or AACVPR certification: CCRP, EP-C, CEP, or RCEP • Experience working in a clinical or rehabilitation setting, ideally with cardio-pulmonary patients • Experience leading group classes or working as a Group Fitness Instructor • Knowledge of cardiovascular/pulmonary disease management • Knowledge of exercise prescription and behavior change techniques • Ability to explain complex medical information understandably • Ability to work independently and as part of a team • Strong organizational and time management skills • Empathy and patience for patients with varying physical and emotional challenges
