Beschreibung

• Conduct secondary clinical chart reviews, including concurrent and retrospective reviews of defined or targeted patient populations • Identify opportunities for documentation clarification, principal-diagnosis accuracy, additional comorbidity capture, mortality documentation, HACs, PSIs, and other priority diagnoses • Resolve DRG, coding, and other documentation discrepancies • Collaborate with the coding department and Clinical Documentation Specialists to reconcile code and DRG assignments • Communicate review findings to Clinical Documentation Specialists for follow-up and query initiation • Query providers using the concurrent query process and ACDIS/AHIMA compliant query-writing guidelines • Educate CDI staff and clinical personnel on documentation opportunities and CDI program goals • Work directly with physicians and other clinicians to improve documentation quality and completeness • Collaborate with Compliance, Revenue Cycle leaders, providers, quality, case management, and coding teams • Analyze and interpret clinical data to identify documentation gaps, inconsistencies, and improvement opportunities • Ensure documentation is clinically appropriate, accurately reflects severity of illness and risk of mortality, supports accurate coding and billing, and complies with CMS and other regulatory standards

🎯 Anforderungen

• Bachelor’s degree required • Graduation from an accredited School of Nursing; BSN or bachelor’s degree in a health-related field preferred • Minimum of 3–5 years as a Clinical Documentation Integrity Specialist required • Strong clinical and critical-thinking skills • Strong understanding of disease processes, clinical indications, and treatments • Understanding of provider documentation requirements for severity of illness, risk of mortality, diagnoses, and procedures • Knowledge of Medicare, Medicaid, and commercial payor rules • Understanding of hospital-acquired conditions (HACs), patient safety indicators (PSIs), and mortality models • Experience with encoder and DRG assignments (MS and APR) • Working knowledge of Official Coding Guidelines, Coding Clinic, and federal DRG-system updates • Currently licensed or licensed by endorsement as a Registered Nurse, MD, or MD equivalent • CDIP or CCDS certification required or obtained within six months • Experience with ACDIS/AHIMA compliant query-writing guidelines
