Beschreibung

• Coordinate follow-up care plan needs by scheduling appointments or enrolling members in programs • Assess member compliance with medical treatment plans via telephone or on-site visits • Identify barriers to plan compliance and coordinate resolutions • Identify opportunities affecting quality goals and recommend process improvements • Recommend treatment plan modifications and determine the need for additional services with case management and providers • Coordinate identification of and referrals to local, state, or federally funded programs • Coach members on ways to reduce health risks • Prepare reports documenting case and compliance updates • Establish and maintain relationships with agencies identified in appropriate contracts

🎯 Anforderungen

• H.S. diploma or equivalent • Minimum of 1 year related experience • Any combination of education and experience which would provide an equivalent background • Certified nurse assistant or certified medical assistant preferred • BS/BA degree in a related field preferred • Bilingual candidates preferred • Proficiency in Microsoft Suite applications • Eastern time zone preferred • Candidates must reside within a reasonable commuting distance from the posting location(s), unless an accommodation is granted as required by law • New hires in certain patient/member-facing roles must become vaccinated against COVID-19 and Influenza, unless an acceptable explanation is provided
