Beschreibung

• Build meaningful relationships with patients and families • Understand patients' goals, barriers, and motivators • Create personalized care plans addressing medical, behavioral, social, and emotional needs • Coordinate care across providers and settings • Ensure safety, quality, and continuity of care • Educate and empower members to make informed healthcare decisions • Help members navigate the healthcare system • Collaborate with an interdisciplinary team • Share insights and advocate for whole-person health • Use data to identify opportunities and measure impact while keeping patients at the center • Maintain compliance with regulatory standards • Participate in team activities, training, and development requiring minimal travel

🎯 Anforderungen

• Minimum of 5 years as a Registered Nurse • Active RN license: compact, multi-state licensure • Strong background in care management • Experience with complex and chronic condition management • Skilled in motivational interviewing • Skilled in patient education and patient advocacy • Ability to work independently in a remote environment while staying connected to the team • Comfort with technology and multiple EMR systems • Ability to travel 5% to 10% for team activities, training, and development • Employees provide their own internet connection • Internet speed of at least 100 Mbps download and 10 Mbps upload • Bachelor's Degree in Registered Nursing is preferred • Specialty Certification in Care Management (CCM) is preferred • Multilingual ability is a plus • Must follow security policies and procedures protecting PHI, PII, and company intellectual property • Must disclose any secondary employment • Must be accessible during stated working hours • Must participate in virtual meetings with cameras on
