Beschreibung

• Provide integrated behavioral health care management for Illinois Medicaid members • Serve as the primary point of contact for members through face-to-face and telephonic engagement • Deliver personalized care coordination, crisis intervention, and peer support • Support members with behavioral health conditions, including SMI, SED, SUD, and justice-involved populations • Engage members in their communities and conduct face-to-face assessments • Coordinate behavioral health and medical services and support treatment-plan adherence • Improve health literacy and address health-related social needs • Lead interdisciplinary care teams and oversee care planning, transitions, and service delivery • Facilitate interdisciplinary care team meetings among providers, Service Coordinators, and Care Management Extenders • Support transitions of care • Collaborate with internal departments, providers, and community-based organizations • Follow IDHS, CMS, and NCQA regulatory compliance processes • Travel up to 25% within Illinois for collaboration, meetings, and field interactions

🎯 Anforderungen

• Applicants must reside in Cook or Kane County, Illinois, within the listed ZIP codes or within a 10-mile radius • Active Illinois license in LCSW, LMFT, or LCPC; no supervisees or provisional licenses • 2+ years of post-degree clinical experience in a behavioral health setting • Case management experience working with complex SMI, SUD, and SED populations • Ability to travel to region-based facilities and homes for face-to-face assessments • Ability to use electronic information applications/software, including electronic medical records • Intermediate to advanced computer skills • Experience with Microsoft Word, Outlook, and Excel • Valid driver's license, car insurance, and reliable transportation • Ability to work Monday–Friday, 8:00 a.m.–5:00 p.m. Central Standard Time • Ability to meet remote-work internet requirements: at least 25 Mbps download and 10 Mbps upload • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information • Must complete tuberculosis screening if selected • Must maintain personal vehicle liability insurance under the driver safety program
