Beschreibung

• Perform duties as a patient navigator guiding incoming calls for new patients into the program • Provide patient education and ongoing continuity of care for patients • Collaborate with healthcare providers to coordinate patient care • Communicate with patients regarding their care via mail, telephone, and in person • Coordinate patient activities such as support groups and educational series • Ensure timely follow-up and surveillance of patients • Assist in developing efficient patient navigational procedures

🎯 Anforderungen

• Minimum of 2 years LPN work experience (Required) • Graduate from Specialty Training Program-Nursing (Required) • Licensed Practical Nurse - Default Issuing Body (Required)
