Beschreibung

• Provide mental health rehabilitation services and supports to adults and families • Decrease hospitalization and crisis episodes and increase community functioning • Support clients in achieving rehabilitative, resiliency, and recovery goals • Provide continuity of program services in a safe, therapeutic, ethical, legal, and moral manner • Participate as a team member in delivering mental health services to clients and families • Facilitate recovery issues in clients’ daily living situations • Undertake primary case management responsibilities for designated clients • Conduct individual, group, and family counseling and educational sessions • Maintain client service records consistent with regulations • Communicate with admissions, process, and discharge planning staff • Coordinate services from community resources • Use treatment concepts, evidence-based practices, and service modalities • Present workshops, lectures, and training to clients, families, staff, and community groups • Make court appearances on behalf of Rosecrance clients when needed • Participate in Clinical Team meetings and activities • Maintain confidentiality under the professional Code of Conduct and applicable law • Deliver exceptional customer service and represent the agency positively • Perform other related responsibilities delegated by management

🎯 Anforderungen

• Bachelor degree in Psychology, Counseling, Social Work, or other related field, or equivalent experience and training • Valid driver’s license and ability to meet Agency insurance requirements • Must have own transportation • Self-starter with ability to work independently and with a team • Computer proficiency required • Ability to navigate and document in electronic healthcare records required • Ability to accurately complete required documentation within the time frames • Ability to motivate and problem solve regarding effective clinical practices • Ability to adapt verbal and written communication to the language and style of the client and their family • Familiarity with court proceedings and availability to make court appearances on behalf of Rosecrance clients • Must pass an occupational health screening • Must work Monday-Friday, 8:30am-5:00pm
