Beschreibung

• Provide oversight and/or outreach, engagement, and support for members receiving Tailored Care Management • Facilitate comprehensive assessments of members' needs and evaluate results • Initiate and maintain integrated care plans covering physical health, mental health, social support, lifestyle, community involvement, and risks to well-being • Facilitate regular face-to-face and telephonic meetings with members • Consult with providers to gather and share information relevant to members' care • Serve as the single point of contact for members • Work with multidisciplinary care teams, including primary care providers, specialists, care manager extenders, transition staff, and caretakers/legal guardians • Work remotely between 8:30 a.m. and 5:00 p.m., travel within the community to meet members, and attend meetings as needed

🎯 Anforderungen

• Must meet Qualified Professional (QP) status • Fully licensed by the NC governing board regulating Human Services professions; OR fully licensed RN licensed to practice in North Carolina by the NC Board of Nursing and four years of full-time MH/DD/SA population experience; OR qualifying master's degree in a human service field with one year of full-time relevant experience; OR bachelor's degree in a human service field with two years of full-time relevant experience; OR bachelor's degree in another field with four years of full-time relevant experience • Substance abuse professionals must have the specified supervised alcoholism and drug abuse counseling experience • If serving LTSS members, at least two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience • Must have a valid driver's license • Must reside within Trillium's Mid State Region: Anson, Guilford, Montgomery, Randolph, or Richmond counties • Must be able to travel within catchment as required • All candidates must pass a drug test as a condition of employment
