Beschreibung

• Serve as the primary coordinator for clients with complex needs • Partner with healthcare providers, specialists, pharmacists, social service agencies, caregivers, family support persons, and other Care Team stakeholders • Manage client cases and coordinate healthcare benefits • Provide education and facilitate timely, cost-effective access to care • Promote wellness, recovery, independence, resilience, and member empowerment • Assess member needs across physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community-based services • Oversee client care plans and goal setting • Offer office-based, telehealth, or field-based services where members reside, seek care, or can access services most easily • Connect clients to needed social services and supports • Advocate for clients with healthcare professionals • Use evidence-based practices including Motivational Interviewing, Harm Reduction, and Trauma-Informed Care • Conduct outreach and engagement and log activity in the CRM system • Evaluate client progress and update SMART goals • Provide mental health promotion • Arrange transportation • Complete documentation and outcome measures within care-plan timeframes • Maintain current patient health records in EMR and other business systems • Complete monthly reporting for program compliance • Attend assigned training

🎯 Anforderungen

• 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations • Willingness and ability to work Monday–Friday, 8:30 a.m.–5:00 p.m. Pacific Time, with possible evenings and weekends • Working knowledge of government and community resources related to social determinants of health • Clean driving record • Valid driver's license • Reliable transportation • Excellent oral and written communication skills • Positive interpersonal skills • General computer skills • Working knowledge of Google Workspace, Microsoft Office, and the internet • Must be based in Arizona, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, Utah, or Washington • Must be legally authorized to work in the United States • Influenza vaccination required for patient-, client-, or customer-facing roles, subject to accommodation requests • Background check, potentially including drug test or other health screenings, required before employment • Identity and work eligibility verification through E-Verify upon hire • Bilingual English/Spanish preferred
