Description

• Provide clinical oversight and management of Enhanced Care Management staff addressing clients’ medical, behavioral health, and social needs • Communicate with ECM staff and occasionally patients by telephone, telehealth, and Interdisciplinary Care Team meetings • Discuss medication management and adherence and ensure appropriate follow-up with primary care, behavioral health, and specialist providers • Guide crises encountered by ECM staff • Collaborate with clients’ broader care teams to advocate for clients and ensure needed services • Measure progress toward goals in individualized, patient-centered care plans • Monitor team progress, coach staff, and manage performance as needed • Supervise approximately 10–15 direct reports • Oversee client outreach, enrollments, and creation of Individual Care Plans • Assess whether client needs can be managed by an ECM Lead Care Manager and serve as an escalation point • Drive the team to meet KPIs • Collaborate with Enhanced Care Management Directors on changes and process improvements • Provide clinical oversight approximately 30% of the time, including clinical staff supervision, clinical sign-offs, care-plan oversight, IDT meetings, and consultation on complex cases • Provide direct care management as needed • Travel up to 20–25% for training, meetings, and other business needs

🎯 Exigences

• Active, unrestricted LCSW or LMFT license in California required • Willingness to obtain additional state licensure as needed • 2 years of people management experience (4 years preferred) • 2–3 years of care management experience • 1–2 years of post-acute experience • Willing and able to work Monday through Friday, 8:30am–5:00pm Pacific Time, with flexibility for potential evenings and weekends • Proven, results-driven leadership experience with change management in rapid-growth environments • Strong organizational and independent working skills • Proficiency in Microsoft and Google applications • Salesforce experience is a plus • Excellent communication and motivational skills, including conflict resolution techniques • Working knowledge of Lean Six Sigma methodology is preferred • Must be based in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, Utah, or Washington • Legally authorized to work in the United States • Must complete background screening prior to employment • Patient-, client-, or customer-facing employees must be vaccinated against influenza, subject to accommodation review • Identity and work eligibility verification through E-Verify upon hire • Ability to travel up to 20–25% for training, meetings, and business needs
