📋 Description
• Perform skilled nursing visits in patients' homes • Monitor and report changes in patient condition • Administer medications and treatments as ordered • Provide patient and caregiver education • Perform wound care and other skilled nursing procedures • Complete accurate and timely clinical documentation • Coordinate care with physicians and interdisciplinary team members
🎯 Requirements
• Current California LVN License in good standing • Current BLS Certification • Valid California Driver's License and reliable transportation • Strong clinical, communication, and organizational skills • Home Health experience preferred, but not required
