đź“‹ Description

• Review clinical documentation for coding accuracy and completeness • Evaluate whether assigned codes are supported by underlying documentation • Identify coding inconsistencies, omissions, unsupported assignments, and documentation gaps • Apply current ICD-10 coding standards across assigned cases • Review clinical notes, summaries, and supporting documentation • Validate alignment between clinical documentation and assigned diagnosis or procedure codes • Flag cases requiring clarification or additional documentation • Annotate clinical documentation according to detailed project guidelines • Categorise documentation and coding issues and provide clear written explanations • Evaluate generated clinical notes and summaries against professional coding and documentation expectations • Review multilingual clinical material and identify linguistic discrepancies affecting clinical or coding interpretation • Contribute professional input to coding and documentation review guidelines • Raise clarifying questions and help improve documentation and annotation standards

🎯 Requirements

• Active certification in at least one of the following: CCDS, CHC, CCS, CPC • Hands-on ICD-10 coding experience • Currently working in medical coding, clinical documentation improvement (CDI), or a closely related clinical documentation role • US-based • Advanced C1-level or higher proficiency in English • Advanced C1-level or higher speaking, listening, and writing proficiency in at least one additional language • Strong understanding of clinical documentation and coding integrity • Able to follow detailed review and annotation guidelines consistently • Strong written communication and attention to detail • Additional certifications such as CRC, CDIP, or similar credentials are advantageous • Familiarity with Epic is preferred • Experience with HCC or risk-adjustment coding is advantageous • Previous clinical annotation or model-evaluation experience is preferred • Work must be completed without using confidential or proprietary information belonging to any employer, client, healthcare organisation, institution, patient, or other third party • H1-B and STEM OPT support is unavailable for this engagement
