Absolute Elder Care is a geriatric-focused mobile physician practice that brings primary and procedural medical services to seniors who have difficulty leaving their residences. They provide physician, APRN, PA, podiatry and support-staff care in Assisted Living Communities, Independent Senior Living, Skilled Nursing Facilities and private homes across Central Florida, perform onsite procedures (injections, biopsies, cyst and ingrown toenail removals, etc. ), accept Medicare and multiple insurance plans, and offer help finding assisted/independent living. The practice emphasizes patient-centered, in-facility and home-based care for older adults and has over a decade of experience serving multiple Florida counties.

📋 Description

• Conduct comprehensive assessments of clients' health care needs and develop individualized care plans • Provide ongoing monitoring and evaluation of clients' health status and make necessary adjustments to care plans • Educate clients and their families on disease management, medication management, and other health-related topics • Collaborate with other health care professionals, such as physicians and therapists, to ensure coordinated and comprehensive care • Serve as a resource and advocate for clients and their families, addressing any concerns or questions they may have • Stay up-to-date on industry trends and best practices in senior care to continuously improve our services • Utilize telehealth technology to conduct virtual consultations with clients and their families • Document all client interactions and maintain accurate and up-to-date records • Participate in team meetings and trainings to share knowledge and expertise with colleagues

🎯 Requirements

• Active nursing license in good standing • Minimum of 3 years of experience in a clinical setting, preferably with a focus on senior care • Strong assessment and care planning skills • Excellent communication and interpersonal skills • Ability to work independently and manage a caseload effectively • Proficiency with technology and ability to conduct virtual consultations • Compassionate and empathetic attitude towards seniors and their families • Willingness to travel for occasional in-person visits with clients (if needed) • Availability to work full-time, Monday-Friday during regular business hours
