• Perform telephonic comprehensive medication reviews • Update patient profiles based on information gathered during the review • Discuss any Drug Therapy Problems to optimize patient’s drug therapy and minimize the potential for adverse drug reactions • Work with multidisciplinary team to enhance and develop our Optimize software • Complete at least 2 CMRs per hour • Assist Clinical team in meeting proficiency and quality goals • Establish positive partnerships across business groups both inside and outside of the organization
• Doctor of Pharmacy degree, or equivalent • Active, unrestricted Pharmacy license • Secure high-speed internet access (Preferable Ethernet connection) • Must speak Mandarin or Cantonese • Strong oral and written communication skills • Outstanding telephone skills and etiquette • Excellent clinical and analytical skills • Ability to work independently and show initiative • Working knowledge of Google Docs and Microsoft Excel • Excellent attention to detail and ability to follow procedures
