📋 Description

• Directly supervise a multidisciplinary team including Nurse Practitioners, Registered Nurses, Care Navigators, and LVNs • Hire, onboard, train, mentor, manage performance, implement corrective actions, and terminate staff when appropriate • Lead team meetings, one-on-one check-ins, and performance evaluations • Oversee daily operations of field-based care programs across medical, behavioral, and social domains • Ensure contractual SLAs and KPIs are met or exceeded • Implement performance dashboards and quality assurance protocols • Design and administer a palliative care program for members with advanced or end-of-life conditions • Partner with senior leadership to align program goals, allocate resources, and execute performance, quality, and outcome strategies • Participate in health plan-led patient rounds for complex cases • Provide clinical leadership and consultation for complex case management and care planning • Guide evidence-based protocols, motivational interviewing, trauma-informed care, and risk stratification • Support transitions of care and appropriate service referrals • Ensure HIPAA compliance, patient data confidentiality, audit readiness, and corrective actions • Lead quality improvement initiatives across care coordination • Act as primary liaison for payers, providers, and community partners • Represent APG at health-related events, coalitions, advisory boards, and business development engagements • Collaborate with public health agencies and community-based organizations to expand access to services • Perform other duties as assigned by management

🎯 Requirements

• Four (4+) years of nursing experience, preferably in population health, care coordination, or community-based programs • Two (2+) years of progressive leadership experience in population health, care coordination, or community-based programs • Demonstrated success in hiring, training, supervising, and managing performance of multidisciplinary teams • Deep understanding of SLAs, KPIs, Medi-Cal and Medicare Advantage programs, and palliative care operations • Strong knowledge and application of HIPAA and patient privacy standards • Excellent analytical, problem-solving, communication, and interpersonal skills • Proficient in EMR platforms (Salesforce preferred) and Microsoft Office Suite • Willingness and ability to travel frequently in-state and out-of-state as required • BSN required • Active, unrestricted Nurse Practitioner (NP) license • Certification in Case Management (CCM) and Palliative Care (e.g., ACHPN) required within 2 years of position starting • Current BLS certification • Current and clear driver’s license • Proof of automobile insurance • Ability to physically perform essential duties, including lifting 30 lbs., transporting materials, stooping, kneeling, crouching, reaching, balancing, walking, standing, talking, hearing, and typing
