📋 Description

• Generate, capture, respond to, and coordinate new patient opportunities across B2B referral sources and direct patient/family inquiries • Operate the agency’s virtual Patient Acquisition Command Center • Conduct AI-powered prospecting and referral-source outreach • Monitor electronic referrals • Respond to leads within 15 minutes • Coordinate patient intake • Build and maintain relationships with hospitals, discharge planners, skilled nursing facilities, rehabilitation facilities, physician practices, outpatient clinics, senior living communities, assisted living communities, elder-law professionals, case managers, social workers, community organizations, and other healthcare referral partners • Research organizations and decision-makers • Maintain referral-source records • Conduct approved outreach and consistent follow-up • Identify opportunities and route qualified conversations to agency leadership or field clinical liaisons • Build and operate the agency’s referral-development engine using AI and approved sales technology • Launch outreach campaigns, manage follow-up sequences, monitor and categorize responses, and route warm opportunities • Support conversion of appropriate referrals and inquiries into assessments and admissions • Work closely with U.S.-based clinical and operations personnel without independently making clinical decisions requiring licensed professional judgment

• Combine the mindset of a Clinical Liaison, Intake Specialist, Business Developer, CRM Operator, AI-Enabled Professional, and Patient Acquisition Specialist • Understand referral relationships • Understand urgency and patient communication • Actively create new opportunities • Document everything in the CRM • Use automation to multiply productivity • Understand that the ultimate business objective is appropriate patient admissions • Research target referral organizations and identify decision-makers • Build, clean, enrich, and segment prospect lists • Use AI to research and personalize outreach • Launch approved cold-email campaigns • Support compliant SMS outreach where appropriate • Manage follow-up sequences and monitor responses • Categorize replies and route warm opportunities immediately • Maintain accurate outreach records in the CRM • Ability to work full-time during U.S. Eastern Time
