📋 Description

• Conduct initial and follow-up telehealth evaluations • Deliver care for substance use and mental health conditions • Prescribe medications as appropriate and answer member questions • Complete documentation in the Healthie EHR • Collaborate with counselors, therapists, peer support specialists, case managers, and the broader care team • Attend occasional scheduled team meetings • Provide evidence-based treatment for individuals with substance use disorders and mental health disorders • Support Affect’s mobile technology and care-team model for real-time, data-driven behavioral healthcare

🎯 Requirements

• Must hold an active, unrestricted license in Iowa • Active and verifiable FNP or PMHNP license in the state(s) where you practice, and in your state of residence if it is not Iowa • At least 2 years of experience, ideally in addiction or mental health treatment; relevant previous experience may be considered for candidates with less than 2 years as an FNP/PMHNP • Experience prescribing Suboxone • Active DEA registration • Willingness to obtain multi-state licensure • Medicaid enrollment is a plus • Empathy for members facing behavioral health challenges • Comfort using technology, including EHRs and mobile apps • Quiet, private office with a reliable internet connection for HIPAA-compliant telehealth visits
