📋 Description

• Deliver high-quality telehealth care focused on weight management, prevention, chronic disease management, and lifestyle interventions (e.g. nutrition, movement, sleep, stress) to older adults (65+). • Provide feedback on the development of clinical protocols, documentation standards, and scalable workflows. • Use EMR for documenting patient information and communicating to other health care providers. • Partner with care coordinators, health coaches, and other clinicians to develop personalized care plans.

🎯 Requirements

• Active and unrestricted NP/PA licenses in multi-states including CA, TX, and/or PA. • 5+ years of clinical experience. • 2+ years experience in telehealth with high proficiency with EMRs & communication tools. • Strong foundation in preventive and lifestyle-focused care including management of weight, hypertension, diabetes. • Experience with GLP-1s. • Excellent communication skills and empathy-driven clinical approach. • Comfortable working independently in a fast-paced startup environment—you thrive in ambiguity and enjoy building systems and processes from the ground up.
