📋 Description

• Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests • Develop personalized treatment plans based on patients' needs and medical conditions • Prescribe nutraceuticals and/or medications and monitor their effectiveness • Collaborate with other healthcare professionals to coordinate patient care • Provide counseling and education to patients on disease prevention and healthy lifestyle choices • Maintain accurate and up-to-date patient records using electronic health record systems • Stay current with medical advancements, research, and clinical trials in the fields of functional, integrative and lifestyle medicine

🎯 Requirements

• At least two years of experience practicing functional medicine and performing functional medicine specialty testing/labs • Experience in functional and integrative medicine settings preferred • NP/APRN – with a concentration in Functional Medicine, Hormone Health, Anti-Aging or Lifestyle Medicine • Must have (or be able to obtain) an IL APRN license and any remote location determined • Strong oral and written communication skills with the ability to collaborate and interact effectively with patients and other providers • Energetic and outgoing personality with a desire to help people • Patient first attitude and ability to consistently provide a 5-star level of service • Knowledge of HIPAA regulations and ability to maintain patient confidentiality
