📋 Description
• Own the Member Journey and Care Coordination for High-Acuity Members • Serve as the primary care coordinator for assigned panel of Medicare Advantage members • Build trusted relationships with members through telehealth outreach • Manage transitions of care for members discharging from hospitals • Ensure all members’ care plans are updated following transitions • Conduct medication reconciliations and monitor clinical status • Escalate clinical concerns and support APC clinical decision-making • Facilitate cross-disciplinary care coordination across the pod • Support care gap closure activities • Manage members with chronic conditions through ongoing monitoring • Document all clinical interactions accurately and timely
🎯 Requirements
• Minimum 3 years of clinical RN experience • Experience managing medically complex, high-risk patient populations • Prior experience in a Medicare Advantage, managed care, home-based care • Experience working in a telehealth or virtual care delivery model • Associate Degree in Nursing (ADN) — BSN preferred • Active, unrestricted Registered Nurse (RN) license in applicable state(s)
