📋 Description
• Own the member journey and care coordination for high-acuity Medicare Advantage members. • Manage transitions of care for members discharging from various inpatient settings. • Conduct medication reconciliations and clinical monitoring. • Serve as a clinical escalation point within the pod.
🎯 Requirements
• Minimum 3 years of clinical RN experience • Experience managing medically complex, high-risk patient populations • Prior experience in a Medicare Advantage or managed care environment • Experience working with telehealth or virtual care delivery models • Associate Degree in Nursing (ADN) required; BSN preferred • Active, unrestricted Registered Nurse (RN) license in applicable state(s) • Current BLS certification • Familiarity with HEDIS, HCC coding, and care gap management preferred • Proficiency with telehealth delivery platforms
