📋 Description

• Manage AltaMed Managed Care participants assigned to the team. • Work closely with providers and nurses to coordinate services and advocate for participants. • Complete daily notes. • Conduct family conferences. • Assist with community referrals. • Maintain regular contact with participants. • Complete incident reporting.

🎯 Requirements

• A bachelor’ or master’s degree in social work from an accredited university is required. • Minimum of 2 years of experience in case management and social advocacy required. • Experience working with the homeless population is preferred. • Managed Care Experience preferred. • Bilingual English/Spanish strongly preferred.
