📋 Description

• Conduct comprehensive evaluations of patients' physical, cognitive, and psychosocial functioning • Collaborate with clients, families, and healthcare professionals to establish therapy goals • Design and implement individualized treatment plans • Provide therapeutic interventions to improve independence in activities of daily living • Use evidence-based techniques, adaptive equipment, and assistive devices • Educate clients, families, and caregivers on therapeutic techniques and safety measures • Coordinate holistic care with physicians, nurses, COTAs, and social workers • Maintain documentation of evaluations, treatment plans, progress notes, and discharge summaries • Stay updated through continuing education and professional development • Adhere to ethical and professional standards, confidentiality, and respectful care

🎯 Requirements

• Minimum one year experience in providing occupational therapy services in an adult home health setting • Bachelor's or Master's degree in Occupational Therapy from an accredited program • Texas State License as an Occupational Therapist • Knowledge of Kinnser/Wellsky EHR • Experience with communication apps such as Slack • Valid CPR BLS certification • Ability to perform comprehensive assessments, develop treatment plans, and deliver evidence-based interventions based on individual clients' needs • Excellent interpersonal and communication skills • Bilingual English/Spanish preferred or advantageous
