📋 Description
• Serve as primary point of contact for newly referred patients • Conduct outbound outreach to explain Remote Patient Monitoring programs • Engage patients telephonically to establish rapport and encourage participation • Document patient information in the electronic health record (EHR) • Collaborate with nurse care managers to support patient care goals • Respond to inbound inquiries and initiate communications with patients and providers
🎯 Requirements
• Completion of a Medical Assisting program accredited by CAAHEP or ABHES • At least six (6) months of experience performing general duties as a Medical Assistant or three (3) years experience working in a clinic or comparable healthcare setting with direct patient contact • Knowledge of medical terminology and prior experience using electronic health record (EHR) systems • Bilingual (Spanish) preferred • Active CNA certification or equivalent certification (e.g., Pharmacy Technician), in good standing
