📋 Description

• Conduct outbound outreach to newly referred patients and explain Remote Patient Monitoring programs, services, and benefits • Complete telephonic interventions, including Welcome Calls and Post-Physician Visit Updates • Establish rapport with patients, answer questions, and encourage participation and adherence • Document patient information accurately in the electronic health record • Collaborate with nurse care managers and the broader clinical care team to support patient care goals • Track hospitalizations and notify nursing staff upon patient discharge • Coordinate patient assistance services and follow up to confirm services are initiated and meeting expectations • Respond to inbound patient, client, and provider inquiries and determine service needs • Communicate by phone, fax, and email with patients, clients, physician offices, and care providers • Maintain accurate provider contact information • Escalate unresolved patient issues and care-coordination concerns to leadership • Meet established performance metrics and service-level agreements, including call-volume expectations and disengagement thresholds • Comply with company policies, HIPAA requirements, and applicable regulations

🎯 Requirements

• Strong organizational, time-management, and multitasking skills • Excellent verbal and written communication skills with strong attention to detail • Ability to actively listen and engage patients with empathy and professionalism • Ability to work independently in a remote environment and maintain accountability • Strong problem-solving and critical-thinking skills • Ability to build rapport and maintain productive telephonic relationships • Comfort navigating multiple systems and technologies simultaneously • Bilingual Spanish preferred • Completion of a Medical Assisting program accredited by CAAHEP or ABHES preferred • At least six months of experience performing general Medical Assistant duties, or three years of experience in a clinic or comparable healthcare setting with direct patient contact • Knowledge of medical terminology • Prior experience using electronic health record systems • Strong technical aptitude, including assisting patients remotely with health devices and troubleshooting basic issues • Ability to type a minimum of 50–60 words per minute while speaking with patients by phone • Ability to work schedules aligned to client-specific time zones • Active CNA certification or equivalent certification, such as Pharmacy Technician, in good standing • Dedicated private, quiet, secure home office • Reliable internet access meeting company minimum technical requirements • Ability to tolerate extended computer and telephone use
