📋 Description

• Assess patients’ medical and mental health needs and provide plans of care that proactively manage conditions and address barriers to care • Ensure compliance with local, state, and federal agencies related to clinical services • Prescribe medications and order lab work, diagnostic procedures, and consultations • Monitor patients’ compliance and response to treatment and modify treatment plans • Work with RN case managers as part of an integrated care team • Provide personalized, coordinated healthcare for long-term care residents enrolled in special Medicare Advantage plans • Treat patients with acute and chronic conditions using independent judgment, consulting with a supervising physician as appropriate • Collaborate with facility partners and health plan leadership to implement the national model of care • May provide training and advice to facility staff

🎯 Requirements

• FNP, AGNP, AHACNP or PA license required • Degree from an accredited APRN or PA program • 3 years’ experience in clinical nursing or rehab in geriatric populations • Electronic Health Records experience • Working knowledge of Microsoft applications, including Word, Outlook and Excel • Masters or better • Experience in primary care in geriatric medicine preferred • 3 years’ experience in long-term care preferred • APRN Advanced Practice RN certification/license required
