📋 Description

• Assess patients’ medical and mental health needs and provide proactive plans of care • Address barriers to care and manage patients’ acute and chronic conditions • Provide personalized, coordinated healthcare to long-term care residents • Prescribe medications and order lab work, diagnostic procedures and consultations • Monitor patients’ compliance and response to treatment and modify care plans • Collaborate with RN case managers as part of an integrated care team • Work with facility partners and health plan leadership to implement TruHealth’s national model of care • Consult with a supervising physician as appropriate • Provide training and advice to facility staff when required

🎯 Requirements

• FNP, AGNP, AHACNP or PA license required • Degree from an accredited APRN or PA program • 3 years’ experience in clinical nursing or rehab in geriatric populations • Primary Care in Geriatric Medicine experience preferred • Long Term Care experience preferred • Electronic Health Records experience • Working knowledge of Microsoft applications, including Word, Outlook and Excel • Ability to assess and treat acute and chronic conditions • Ability to prescribe medications and order lab work, diagnostic procedures and consultations • Ability to work in compliance with federal and state regulations
