📋 Description

• Provide personalized, coordinated healthcare to Medicare Advantage plan members in nursing homes or assisted living communities • Assess patients’ medical and mental health needs and provide proactive plans of care • Manage acute and chronic conditions using independent clinical judgment • Ensure compliance with local, state, and federal clinical-service regulations • Prescribe medications and order laboratory work, diagnostic procedures, and consultations • Monitor treatment compliance and patient response, modifying care plans as needed • Collaborate with supervising physicians, RN case managers, facility partners, and health plan leadership • Implement TruHealth’s national model of care • Provide training and advice to facility staff when required • Help improve residents’ quality of life and prevent unnecessary hospital visits

🎯 Requirements

• FNP, AGNP, AHACNP or PA license required • Degree from an accredited APRN or PA program • 3 years’ experience in clinical nursing or rehab in geriatric populations • Electronic Health Records experience • Working knowledge of Microsoft applications, including Word, Outlook and Excel • Advanced practice provider experience as a nurse practitioner or physician assistant • Experience in primary care or internal medicine preferred • Geriatrics focus preferred • Masters or better • APRN Advanced Practice RN certification/license required
