📋 Description
• Assess client needs, provide guidance to health services, and maintain regular follow-up • Offer informal support and education while assisting clients with appointment scheduling, attendance, and program applications • Build key relationships with internal teams and external community agencies/referral sources • Document client interactions, services, and outcomes to maintain accurate records
🎯 Requirements
• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required • Preferred BLS Provider certification; American Heart Association or American Red Cross accepted • Preferred driver's license
