📋 Description

• Initiate virtual appointments with patients to collect health care information prior to the visit with MDs, Nurse Practitioners, and Licensed Clinical Social Workers • Help to coordinate treatment with the patient, family, and the multidisciplinary care team • Support staff to complete tasks such as vital sign collection, consent collection, recording, and scheduling of future appointments • Work as part of a multidisciplinary care team composed of psychiatrists, nurse practitioners, social workers, and Facilitated telehealth coordinators • Ensure smooth patient and workflow within the department and assist the patient care team in implementing the medical plan of care

🎯 Requirements

• At least 1 year of experience directly related to the duties of the position • Minimum of two years’ experience in a medical setting or one year experience in a behavioral health setting, or a comparable job classification • Prior experience working in behavioral health and/or with an elderly population is preferred • Bilingual (Spanish-English or Haitian-Creole) preferred • Graduate and completion of an approved Medical Assistant Program
