📋 Description

• Support members through the Heart Wise program, a structured 4–6 week virtual program to improve blood pressure • Provide nutrition education through telephonic and video visits • Establish SMART goals, monitor progress, and guide lifestyle changes supporting heart health and blood pressure management • Manage a complex patient caseload through comprehensive nutritional assessments and nutrition-focused physical exams • Develop and manage nutrition care plans and monitor outcomes • Provide patient and family nutrition counseling and education • Develop Nutrition Standards of Practice and educational materials with peers • Serve on the interdisciplinary healthcare team and contribute to patient care policies, programs, and quality improvement activities • Maintain competency in enteral, parenteral, and oral nutrition management protocols • Initiate and manage nutrition care, including nutrition support, according to protocols and state regulations • Evaluate care plan effectiveness and communicate with healthcare team members • Document in medical records, patient plans of care, and clinical pathways while meeting regulatory requirements • Provide nutrition education to patients, families, staff, and healthcare professionals • Provide leadership, monitor patient care procedures, and liaise with nursing areas • Participate in process improvement teams, continuous quality improvement activities, and outcome studies • Mentor new staff and interns where assigned • Interact with patients, families, healthcare providers, and nutrition and dietary staff • Work independently and provide guidance to nutrition services staff • Provide community education as needed

🎯 Requirements

• Bachelor's degree in Nutrition, Dietetics, or equivalent coursework • Must be a Registered Dietitian or RD Eligible based on academic and professional requirements defined by the Commission on Dietetic Registration (CDR) • Must possess RD credentials within 6 months of hire • Must hold current registration through the Commission on Dietetic Registration of The Academy of Nutrition and Dietetics • Must maintain ongoing registration verification annually • Current licensure as required by State law • Effective January 1, 2024, CDR requires a minimum of Master's degree to be eligible to take the registration examination • Must communicate effectively and function independently • Must be effective at prioritizing assigned patient caseload • Completion of post-offer Occupational Health physical assessment, drug screen, and background check is required • Arizona Fingerprint Clearance Card and Arizona Criminal History Affidavit required for specified behavioral health settings
