📋 Description

• Receive and coordinate hospice referrals from physicians, hospitals, facilities, and community resources • Lead client pre-admission activities and referral management processing • Coordinate hospice services with referral sources, insurance companies, clients, and families • Process referrals and admissions, collaborate with branch operations and sales, and enter admission data after requirements are met • Obtain billing, clinical, referral-source, and account information • Screen and verify benefits under Medicare, Medicaid, and third-party payer guidelines • Coordinate equipment with contracted vendors and gather admission documentation • Monitor referral calls, fax lines, and portals • Contact clients, families, program managers, liaisons, and referral sources as needed • Enter information accurately into HCHB • Verify insurance eligibility and authorization • Explain hospice benefits, services, and philosophy to clients and families • Schedule hospice informational meetings • Coordinate durable medical equipment and comfort kits for client transitions • Audit HCHB charts and ensure documents are signed, dated, and uploaded • Follow up on pending referrals and maintain relationships with referral sources • Support marketing strategies and site/company growth • Provide billing and insurance documentation to the Billing Coordinator • Maintain communication with pending clients, families, and referral sources • Attend assigned in-services and continuing education programs • Maintain HIPAA privacy and confidentiality • Perform related duties as assigned by the supervisor

🎯 Requirements

• Four (4) year college degree • Minimum two (2) years of experience and/or training with hospice • Experience in a medical office environment, customer service role, and intake or inside sales experience are preferred • Licensed Practical Nurse (LPN) certification is preferred but not required • Excellent verbal and written communication skills • Ability to work in a demanding, high-stress environment • Ability to sit, stand, bend, lift, and move • Ability to comply with occupational safety requirements • Confidence discussing hospice benefits and obstacles to choosing hospice • High integrity, negotiation, advanced customer service, organizational, and interpersonal skills • Ability to collaborate with clinical, sales, and non-clinical staff • Ability to work with physician offices, hospitals, and facility administrators • Ability to learn and utilize HCHB, including hospice workflow tasks • Critical thinking and ability to manage multiple tasks and priorities • Willingness to learn hospice regulatory standards, including Medicare Conditions of Participation • Extensive knowledge of hospice benefits, services, and philosophy • Effective communication with hospice clinical/operations, sales, referral, and healthcare personnel • High attention to detail • Ability to cope with work involving ill, disabled, and dying clients • Proficient in Microsoft Office Suite, including Excel and Teams • Ability to read, write, and effectively communicate in English • Availability to work Saturdays and Sundays, at least one major holiday, and two minor holidays per year
