📋 Description

• Provide personalized home health care to geriatric patients in their homes or residential facilities • Work closely with the Director of Nursing to develop individualized care plans • Support each patient's unique goals and needs • Complete 45–60 minute visits during 9-week home health episodes of care • Provide additional compassionate hospice services • Travel throughout the service area and assigned coverage territory • Participate in one weekend on-call rotation per month • Deliver high-quality, patient-centered care to rural Arizona communities

🎯 Requirements

• Active Arizona RN License • Valid Driver's License • Current Auto Insurance • Arizona Level One Fingerprint Clearance Card • Current CPR or BLS Certification • Current TB Test (skin test within 1 year or chest x-ray within 5 years) • Willingness to travel throughout the service area • Ability to provide coverage in the Peoria Buckeye area • Availability to take one weekend on-call rotation per month • IV experience preferred • Experience providing home health or hospice nursing care with geriatric patients
